Abstract


In a patient with moderate or severe crowding and protrusive lip, premolar extraction has usually been considered
to provide required space for alignment. However, non-extraction treatment has been increased in accordance with
advancement of biomechanics, application of various orthodontic devices, and widespread use of mini-implants,
recently. Distalization of the entire dentition by using orthodontic mini-implants and interproximal reduction (IPR) can
be an efficient approach to relieve crowding and improve the lip profile. It may also be a good substitute for premolar
extraction. This case report describes the treatment of a 15-year-old female with severe crowding, lip protrusion and
incompetency. Although the crowding in the maxillary arch was severe enough to require premolar extraction, nonextraction treatment was planned except the maxillary left third molar. Distalization of the entire maxillary dentition
with orthodontic mini-implants and IPR could successfully resolve the crowding, lip protrusion and incompetency
